You know those statements sometimes found at the end of operative reports? - the ones where the doctor is asking for more reimbursement because the operation was more difficult than usual.

"Due to the difficulty in deciphering this dictator's words, this report took an hour to transcribe, although ordinarily this type of report would take 10 minutes to transcribe. Therefore, it is a level 5 and I am billing for 3 times my usual rate."
