Lobectomy with T1-T3 "riber" section.  Not sure what this is.  Any help would be appreciated.

I know that sudden dissection can't be right but when transcribing a breast biopsy procedure that what it sounds like.  What else could it be?  It doesn't sound like sharp or blunt....what else could sound like sudden?  Any help would be appreciated.

__ ectomy.  Here is the context:

status post a recent composite resection including a right neck dissection and ___medivalectomy___ and free flap reconstruction with a fibular free flap. 

Maybe my ears are not working right tonight!! Imagine that after being at this for 13 hours today!  Okay - many interuptions (I homeschool too), stopping to fix supper etc.  but needless to say -- I'm tired!!

There is a history of hypertension, (s/l thoratic aortic dissection) ?

I have never done an op note and have certainly never done a groin node dissection!  There are no sample reports available.  I have done this doctor before in his clinical setting and he is one of my favorites, but no op notes - the problem is a very poor quality recording - and in an area that I do not have experience!!  HELP!!  Each line is a separate issue.  Some I just need some verifcation that I did it right.

We dissected the lymph node bearing tissue off of the external __s/l dish__ muscle, exposed the femoral nerve artery and vein. 

We used hemoclips and silk ___ for hemostasis and controlled lymphatic vessels.

We dissected along the superficial femoral artery and femoral nerve to __revolve?__ the lymph node bearing tissue from this area.   

We irrigated __and assured?__ hemostasis

We identified the ureter which was reflected medially.

assured hemostasis placing __ FloSeal __ along the obturator foramen area and there was no sign of bleeding

We closed the fascia with a running loop #1 _PDS__ suture. 

closing the femoral canal defect with an interrupted, figure of 8, Ethibond suture.

is anyone familiar with a procedure for aortic dissection that sounds like bentile or ventile procedure? 

"underoging an emergent bentile procedure for an acute aortic dissection"

Under medical history the doctor says, pretty clearly, "stick left thyroid mass"

I cannot verify this term. Any help appreciated. thank you

1. Hypothyroidism-clinically s/l youth thyroid.  Could this be.....cannot find it in my books!!!! 

There is a past history of hyperthyroidism but TSH on 03/11/2008 was 0.86, indicating a ?? thyroid state.  S/l "new" thyroid state.  Thanks for the help!

A second s/l interbrachiable/interpranchable nodule measuring 1.5 cm * 0.9 * 1.4 cm is present overlying the lower pole of the right lobe.

Doc also talks about interpranchable  mass measuring 3.1 cm * 3.5 * 2.2 is seen overlying the midbody of the right lobe.

This is an ultrasound of thyroid gland..Any help??

Has anybody ever heard this phrase or might be able to take a guess at what he could be saying?

The pathology report shows that over the 3.5-cm left thyroid nodule, there was note of numerous fistula sites, and rare bland thyroid follicle cells and colloid material. 

Not sure if he is actually saying "numerous fistula sites" but that is certainly what it sounds like.  Any suggestions?
