?? lobotomy...

Under physical exam, this patient has bruising on the backs of both hands.

Dictated:  Bruising on the back of both hands, possibly due to attempts at IVs or s/l potts or pos lobotomy. 

I don't think lobotomy makes sense for that either, though.  Any ideas?
