The patient swallowed a fentanyl patch.  This is the discharge summary and in it, the ESL doctor says, "Transaminitis, most probably secondary to N A L D vs Lortab intoxication."  She never says these letters again and all I can document is neonatal adrenoleukodystropy.  Since she's almost 60, I don't think that would fit.

My sister, who is also an MT (with more experience than me), got it in one listen!  Too bad I couldn't get a hold of her earlier.  I'm afraid my s/l wasn't much help.  Thanks to those who put out some ideas though!

Dr. is listing patients PMH which includes microcephaly, hyperlipidemia, dysarthria, hypertension, s/l Laid disorder, cerebral palsy.


At the beginning of the report when he was introducing the patient, he says she has cerebral palsy, seizure disorder and organic brain syndrome.
