drug dosing

What should I do when doc dictates drug dose that is not in drug book.

ex. metformin 875 mg

When I look this up it only comes in 500, 850, or 1000. This is the first time, usually it's 500 or 1000. Should I just change it to 850 or leave it for QA?

Drug dosing q.4h.
Subject: Drug dosing q.4h.

What is the latest AAMT BOS standard for drug dosing such as "q.4h."  I thought I heard that it was changing to "q.4 h. or q. 4h."  Can someone clarify this for me?  Thanks!

Medication dosing
Subject: Medication dosing

If a doctor dictates "Invanz 1 gram q. 24" would you put q.24h.or would you leave it alone.  I know for sure she was scheduled for once a day.  Is there an industry standard on this particular happening or is it the personal preference of each facility?  Thanks!

has anyone ever heard of a word parous or perous???  doc is giving list of meds and says aspirin 81 mg parous or perous daily,  amlodipine 10 mg parous or perous daily.  Definitely not saying p.o. and he keeps saying it, before the word daily everytime.  This is a new one on me. Thanks

Body temperature
Subject: Body temperature

Dictator states the patient's temperature is 75.7.  (Patient is having a surgical consult for abdominal pain).  This is the third time in as many weeks I've had physician dictate a body temperature in the 70s.  Celsius temp is normally in the 30s, Farenheit - of course - is normally in the high 90s.  Am I missing something here?  Is there some other temperature scale I don't know about?  So far I've flagged them as blanks.

Thanks!
