ESL dictator but very clearly says "status post left forearm graft to the pharynx"  The patient has had "tongue and pharyngeal cancer secondary to squamous cell carcinoma" and had a wide lymph node resection of the neck.  I've just never heard of a forearm graft to the pharynx, and will readily admit I still have a lot to learn after 10 years in this profession.  Hope you can help clear this up for me.  TIA
