It should also be noted that studies have confirmed that controlled blood pressure without the use of a thiazide does not resolve it ---is? or as? well of a reduction ---- of the long-term adverse reactions unless there is a thiazide included in the treatment.  The aera I have marked with --- is where I need help. Might have some little word/words wrong here. Sounds like doc has raspy voice while dictating!

I am questioning the last part of the sentence and need help with punctuation there. Does the word debrided go with nail plate or nailbed as tolerated by patient?

I would change it to read:  "His eye contact was intermittent.  Most of the time, his gaze was fixed on whatever toy he was playing with."


Rule of thumb: semicolon: punctuation mark that is used in formal writing between two parts of a sentence, usually when each of the two parts could form grammatical sentences on their own. Also, to be grammatically correct, never end a sentence with a preposition.

CT SCAN cannt figure out part of scan Pls see message

Reveals abnormal hypermetabolism in the lymph nodes in the neck, more right than left including ?factitious learning of the thyroid?, in the anterior mediastinum as well large expansile right rib lesion.  Bone scan reveals uptake in the right lateral 7th and 8th ribs.

but yes there are bloody bowel movements....and occult bleeding would be bleeding from somewhere, and in your case for your question would be the bowel...hemoccult-negative stool test would be no blood in the bowels.

I change elicitable (which to my knowledge has never been a "real word" to "able to be elicited."  My doctor dictator thanked me for this once (I was suprised that he noticed).  I checked this out with several transcripitionists that I have worked with who have lots of experience and are CMTs and they also edit this word to "able to be elicited."
