I'm doing an oncology note on a patient being followed up in the dental clinic , and the dictator says:


The patient returns to our service today for adjustment of maxillary surgical obturator.  Today s/l Cosoft or Cosoff or Kosoft or Kosoff (or Cosopt, but that would be eye drops) was added to the velopharyngeal insufficient area at the posterior soft palatal defect.


I'm out of spelling variations and not having any luck.  I don't know if this is a med or a dressing or what it is.  I'm stumped!
