Please excuse if this is a simple question, but I cannot verify this:

She was recently found to have **interQ-waves** and the computer suggested possible inferior MI on 05/30/06 EKG. She notes easy fatigue and DOE with almost any activity, but denies orthopnea, PND, or angina.

PAST MEDICAL HISTORY: Reduced CEA, HBP, obese, 35% bilateral carotid disease 2003.

He has had quadruple bypass in 1994 and femoral artery bypass in 1999 with carotid artery  CLEAN OUTS and OJEERS as well. 

This does not make sense to me, so any advice appreciated! The pt is seen for chest pain. It is the "ST segment, plane, or depression" that is throwing me off. Could he be saying something other than plane that I am missing? Thanks again!

I have told him at this time, based on the duration of his exercise and no definite ST segment, plane, or depression, that he is in a relatively good prognosis category.

Doc is saying the patient had S/L "cord-atendony" rupture needing open heart surgery to repair that.

Could this be it?

The patient has a history for sudden cardiac death status post AICD placement.  It sounds like he is saying death, but I don't think that makes sense as the patient is still alive ???? Definitely not talking about family history.

The patient is currently admitted ale monitor bed in deecee setting.

underwent percutaneous coronary vessel  s/l  "sum of division times 2" in the right coronary artery ( s/l strugglers and stents).   Thanks this is my first day on this account. 

Doc is saying:

The skin was consistent with s/l "ana lese" single vessel coronary disease. 

Can't figure out what he is saying!!  Please help! 

Results of an exercise sestamibi stress test.....A long (s/l membrative septum) with attenuation artifact over the apex was appreciated.

On 10/07/05, off pump quadruple coronary artery bypass grafting (left internal mammary artery emulon reverse saphenous vein graft).

Abdominal aortic angiography with bilateral lower extremity runoff, endovascular repair of infrarenal abdominal aortic aneurysm using a s/l Cokevenous stent graft system, percutaneous transluminal artery stenting and bilateral common and external iliac arteries. 

She had a stent placed in the RCA.  She also had an OSTEO LEFT MAIN and moderate mid LAD lesion.

I was wondering if the Osteo left main is correct and if so, if that is how it is written.

These two are confusing me!  Are these punctuated correctly??

The report then states correlate clinically for paroxysmal atrial fibrillation, as her diary entry showed sinus rhythm, with questionable episodes of atrial fibrillation.

It did reveal sinus rhythm, sinus tachycardia; suggested clinical correlation for atrial fibrillation, as some of her symptoms occurred with sinus rhythm, with a questionable atrial fibrillation.

Under Impression:  s/l "pixel informationaling" in sinus rhythm with amiodarone.  Also anothe one:   we can restart her s/l ACE or ARP or put her on Bydel".  Any help appreciated!

T-wave abnormality in the S/l three Na-VF appears to be nonspecific.

EKG is either sinus rhythm or a flutter with a 3:1 conduction.

Is it a flutter or A flutter??
