Subject: Diovan HCT

How does one go about transcribing Diovan HCT when the drug is a combination of valsartan and hydrochlorothiazide.  Appendix B of the BOS states that the meaning of HCT is hydrocortisone and HCTZ is hydrochlorothiazide and to spell out both HCT and HCTZ.  I trust this is the correct board to ask this question.  I'm really getting myself confused with this particular drug combination!  in advance!

The doc is saying the pt is on 320/50.  The highest I can find is 320/25, although I did get google hits with patients talking about being on 320/50.  Can anyone verify this?
Under meds, doctor says post Diovan. Is this a term that is used? Haven't heard of it. Thanks.
