Help

I am testing right now and am stuck on one part. Any help would be greatly appreciated!!!

For the heading OBJECTIVE

He starts by saying vitals signs and then going into the weight and blood pressure....

Would you write this like this:

OBJECTIVE

Vital signs: Weight: 143 pounds, blood pressure 120/92, temperature is 98.2.

He then goes into how the patient is in good health.

Any help would be appreciated!
