Does this seem correct I can't verify. After hemilaminectomy it sounds like either "for which is the NI-FOCAL disc herniation by me" or "for which is the FOCAL disc herniation by me. TX for everyone's help
Mr. _____ is a 63-year-old right handed male in status-post right L5-S1. Hemilaminectomy ***for which is the focal disc herniation by me on 8/17/93. I followed him for post-operative course and symptoms of lower back pain, left arm numbness and weakness, as well as a right CVA, and neck problems
