I am sorry if this is simple, but I have never heard it. Doc states that the patient has had **soup-ons** of short-term memory. This is a pain medicine specialist, so not a word I am used to hearing. Thanks to all!

This is in an operative note and he says it (mumbles it) twice, the first time is in the findings:  Confluence **in-sunning thorough**  associated with facet cyst, left-sided, at L5-S1.


The second time is in the body of the OP: There was a confluence of the lateral dura and facet cyst, with effacement of the dura and **in-sunning**. At this point.... All help appreciated!

The interesting thing about it is the patient has (sounds like "a century") of a lot of animals and pets, dogs, cats, and birds, close to his bedroom.


 


Then later she states, "allergic rhino-sinus has something to do with his pet "century." I cannot figure this out! Thanks for looking!

This guy has a thick, possibly Russian, accent.


However, he was evaluated by his urologist, who is following him for prostate problems, and was told that this was not ___ to have some microscopic hematuria.


It sounds like he is saying early, but I am not sure. It has a definite Y sound at the end. Thanks so much!

As an aside, for a laugh, I used to work for a neurologist who apparently had a patient pull a Rx paper off the top of his Rx pad when he wasn't looking.  The doc got a call a few hours later from a pharmacy trying to verify the Rx for "mo feen".

It is hard to differentiate sometimes between certain letters. It is obviously a test of some sort, so maybe CBC, mid-small bowel stool for C-diff, gram stain, culture and sensitivity, ova and parasite? just a thought. trying to help out.
