I can usually get away with this (except one radiologist):

At c3-C4, a 2-mm left paracentral annular bulge and mild central canal stenosis are seen. At c4-C5, a 2-mm central and lateral annular bulge and spur with moderate spinal stenosis are seen.

Good luck. Some radiologists just hate an extra two-letter word or two, darn them! I think verbs and articles make them itch or something.
