On auscultation is it lung "fields or fills"  I know this is such a simple question but I am honestly not sure.  Thanks for the help

I feel silly asking this spelling of a non-medical word, s/l rehesitant is the word and I have noone to turn to right now to ask, so any clues?  I can pronounce it but cannot spell the darn thing.  The patient is resident or rehesitant to surgery.

BTW, "principle diagnosis" is INCORRECT!! That company is way off, unless it's the client that knows no better and is insisting on that spelling.

I have been transcribing for this doctor for some time now, and I swear he is saying


Hydrochlorothiazide 50 mg S/L in-cake.


He has never used this term before, and I cannot figure out if it is an abbrev. for something or what. He never says intake, and he lists 9 other medications in this reports that does not say this. Any help appreciated..maybe I am just hungry lol

Doc is doing paracentral translaminar epidural steroid injection but she usually says paramedian and just wondering if this is a different somehow.
