On the lung exam, the physician is saying "E to A" changes.  The company I work for does not want any abbreviations.  What would be the best way to transcribe this?   

sounds like:

Lungs are clear. No wheezing. (S/L) Yulit squeeze.

any ideas???

On a lung exam it sounds like he is saying no son or soncar basal rales???

There is diminished air movement air movement in all fields, but no clear s/l epintious sounds heard.

Hi.  Doctor is saying what sounds lke:  Lung are clerr "ABCs" with some faint wheezes.  Is that rght? The only remotely similar thing I can find in my acronym book os the CPR ABCs   - airway,breathing & ciculation.  Any help would be greatly appreciaaed.  Thank you!

I would type it this way or at least make it present tense.  On my accounts, I am required to categorize these things. 

PHYSICAL EXAMINATION:

Abdomen:  Significant findings for left lower quadrant tenderness. Vital signs:  Blood pressure 124/78, pulse rate 96, respiratory rate 20. Heart: S1 and S2. Lungs: Clear to auscultation bilaterally.

In the physical exam, s/l thenar and epithilial wasting.  He is listing liver disease stigmata and I just blanked out on this.  Help!

Physical exam, under Extremities, doctor states ___ pulses throughout.

It sounds like temporis pulses. I know of temporal pulses, but would that be throughout the extremities? Thank you!

EXAMINATION:  No costovertebral angle tenderness.  Abdomen has normal active bowel sounds in all quadrants.  It is supple.  Extremely tender left upper quadrant and going down not quite into the groin.  Difficult to check the (s/l sternal) border because of increased tenderness but the spleen may be mildly enlarged but no distinct masses felt.  I am unable to check for rebound or guarding because she is so exquisitely tender with even a very light touch.

ASSESSMENT:  Acute left upper quadrant pain.

s/l sternal border?  Any help would be great?

I am wondering if, in the last sentence, where "drop can arc" is dictated . . . would she be referring to three tests/maneuvers such as Drop Arm, Empty Can, and an Arc test, or is it correct as is?

She has excellent range of motion of the shoulder.  Good grip strength.  Distal pulses and sensation are intact.  She can do a full drop can arc.

HEENT: It sounds like she is saying 'TO' mucosa pink and moist.  Or TL mucosa.

i have never heard this before.  its not a standard.  going through the physical exam as normal and says ....

Eyes:  Split 1 and 2.

Ears, nose, throat:  1,2,5,6.

Neck:  1,2.

Respiratory:  1 and 4. 

Cardiovascular:  2, 3, 4, 5, 6, 7. 

What is this?  I have never seen or heard such a thing before.

Posture and Balance:  Good sitting balance.  Normal truncal tone.  No scoliosis.  No S/L: SAY-COE-ZYM-BOWL.
