Long "a" or short "a"?  Could she be saying "without a [s/l tar]" or "with [s/l atatar]"?  What is the next sentence?  Could there be more to the word that you thought was part of the next sentence?  Just some thoughts, probably not much help.

when doing the Google wildcard search you can use the MINUS(-) exception to narrow the search matches; you can also enter multiple asterisks within the quotes, e.g. "medial * * skin lesion" -biopsy will return all possible matches EXCEPT biopsy; hope that's not confusing...and hope it helps.
