 After informed consent was obtained, the patient's left leg was marked as the preoperative site.  The patient was then taken back to the operating room and placed supine on the operating table.  She was placed under general anesthesia and time-out was performed to verify correct surgical site with surgical team.  The patient was given preoperative antibiotics of Kefzol.  Her left leg was then prepped and draped in the usual sterile fashion.  Fluoroscopy was brought in to identify correct starting site at the anterior femoral neck.  Once the correct starting site was identified, a skin incision was made, and dissection was carried down to the femoral neck.  A guide __________ was then used to find her correct surgical site on the bone again using fluoroscopic guidance.  Once the insertion site was identified, the __________ was driven into the femoral head.  This was checked on fluoroscopy and felt to be in good position.     Ortho: Soft tissue s/l --position fracture hematoma

 After the soft tissue s/l (muffled)position, fracture hematoma, and comminution were removed, reduction maneuver was performed using a large periarticular clamp.

 He was gently injected with 1% lidocaine 2 cc with a tendon sheath type block. He tolerated it well. A pen/pin was placed between the third and long fingers. A closed reduction was performed. Improvement of the reduction was noted although there was still slight deformity.

 The first three times he says it while discussing x-rays and CTs. This shows a compression fracture of L3 with compression of the superior endplate with **Meth-in-fac-late** present within the body, all contained within the body itself.   
 Under impression:  History of a compression fracture with treatment of the meth-in-fac-late at the L3 vertebral body. 
 Under plan: Pedicle screws could not be placed into the pedicles of L3 because of the meth-in-fac-late

 OP report lumbar spine, I cannot find this type of "view." He is either saying "and/or toggle view" or and orthoggle view" along those lines. I hear a defintie th sound, though, and cannot find toggle view.  
 Each screw at L4 and L5 bilaterally were confirmed with radiographic confirmation and **orthoggle ** views.
