Your reference is British. Note the spellings of centre, etc. Check Vera Pyle's book and Dorland's Medical Dictionary and also the American Medical Association reference for making nouns out of proper names. Our own BOS states not to capitalize words derived from eponyms.

Past history would cover you on all corners. Did the doc dictate any history? Unless she has significant OB/GYN pathology, which the doc would probably make note of, vaginal bleeding and "path" history don't go together. Hope that helped.

I will not use it either. I do not think it is professional looking and if I was still going through training, it would not be acceptable to the school either. but my Stedman's spellcheck will not accept it, either.

Not to be rude - it IS coffee grounds that one can vomit and when used as a noun, but when it's used as an adjective, it is coffee-ground emesis....I suppose it could be an either-or so I'm not giving anyone an argument.  Just have seen Stedman's be wrong on many occasions and I don't use Stedmans as a result.
