Patient diagnosis:  Essential tremor most likely longstanding action tremor. 


Family history of cancer, breast cancer.


PMH:  kindey stones, burns on his right hand, tremor in his left hand.


In PLAN, the doctor dictates,


"Because of subjective increase in left-sided weakness, recommend brain MRI.  Also recommend checking TSH, serum protein and immunoprotein electrophoresis in light of family history and the fact that on rare cases, this could be attributed to S/L: his thyroid disease.  Recommend beta blocker. 


I don't see where she mentions him having a thyroid disease, unless I'm missing something....???


Can anyone help?
