I always use the Quick Look Drug Book. It tells you if the drug is a brand name or generic. If it is a generic drug such as verapamil, they will list all of the brand names it goes by. If none of them are what the dictator is staying (i.e. verapamil SR) I would simply transcribe it as dictated "verapamil SR" the pharmacist will know that the SR is for sustained release.

I don't know if I've answered all your questions. If not, feel free to reply and I'll try to do a better job :-)

My Stedman's OB-GYN book shows
