Question about how to type a drug

I have a friend who QA person told she did not type out right and should read: "q.6.h.  According to their guideline book that we have the book says q.6h.  Book shows no period between 6 and h.  Also found another discrepancy in the word gram.  One page says type "g" and the other page has it spelled out in a drug list.  I always type it "g".   If I was my coworker I would question this especially since it has it two different ways in their book.  They are using Book of Style.  I have sent a message asking them to explain this one.   How would anyone else type it?  Thanks

She has been on s/l Pro-max-in and Cipro I.V. therapy..The dr recommended 2 drug I.V. therapy because of the resistant pseudomonas.

When I google it, I'm finding both.  Cannot locate it in a Stedman's book.  Doctor spells with "a" but you can't always go by the spellings!

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN.  Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading.  Hope that helps!

I'm not sure if I'm any were near the right spelling or pronunciation but it is something like pa-nen-ta-meb and Xeloda.


Here's part of the report: Status post left hepatic lobectomy, isolated segment V resection and radiofrequency ablation of five additional lesions with multifocal metastatic colorectal cancer.  She is restarting her penetameb and Xeloda.  she has also had TheraSphere treatment.

Drug? Ever heard of this drug?

Klor-Con SA.  ?      I have heard of Klor-Con, but maybe it just sounds like "SA" after the drug name.  It is in a list of medications and the dose is 10 mEq.

Yes, it used to be Type II, but clarity is more important, so it was changed. Either would be acceptable in transcription, however.

It has always been Merchant view, as far as I know.  All my Sted refs have Mechant view. Though I do not do BOS, I typed ortho for 4 years in a railroad hospital with lots and lots of injuries/surgeries etc.

2-5 or two to five/1 or 2 or one or two depending upon the client's preference.  numbers/hypen, words/words if that makes sense.  Do not hold me to it though, that is just my way.  Clients are happy.
