Actually, the doctor is not correct.  He said retarded not mentally retarded which is derogatory and politically incorrect.  While mental retardation used to be the accepted medical term it no longer is.  The correct term is developmentally delayed.  Unfortunately, if doctors want to sound like stupid, ignorant (insert expletives) we are forced to assist them by typing what they say. 


I do see one exception though that's minor that I got yesterday.  A person is not defined by their illness or disability.  That's why you shouldn't call someone a diabetic patient or an autistic kid.  It should be the patient has diabetes or the child has autism.  So yesterday when I had an ignorant doctor state "this is a Down's Syndrome patient"  I rearranged the sentence which we are allowed to do under some circumstances and made it read "this is a patient with Down's Syndrome."  And he can kiss my grits if he don't like it!

Used in radiofrequency ablation of saphenous vein.  I can hear a "6" but not so sure of the "vent" part.  The "6-vent" sheath was inserted over the guide wire.  Anyone heard of this?

The doctor spells this out more than once during the report but I've never heard this term before, my medical spellchecker doesn't recognize it, and I can't verify it on Google. Any help would be great!

Following the percutaneous intervention and completion arteriogram of the right external iliac artery, the wires catheters were withdrawn. Please note also just prior to the percutaneous intervention, a full dose of systemic intravenous heparin was given and an ACT was maintained above 200. In addition the short 5 French sheath was changed out over a Magic Torque wire to a long 6 French "raabe" sheath. Following the intervention, the long sheath was then exchanged over the same wire to a short 6 French sheath.
All I usually get is Storz (resectoscope, sheath, etc) for uro ops, sorry! Nm

Does this make sense to anyone as part of Physical examination under Neck:  ..demonstrates full range of motion of his neck without pain or "sheath line" pain to palpation?  This definitely sounds like "sheath line" pain to me.  Is there anything I'm missing that would sound like this??

The carotid sheath is an anatomical term for the fibrous connective tissue that surrounds the internal carotid artery and related structures in the neck.
