Subject: Side effects of fluoxetine

Okay, here's the sentence...

"The S/L: TENDER effects and possible side effects of this medication were reviewed with him including S/L: INTENSIVE FOR NUANCE AT suicidal ideation, but since he has taken this medication before and not had that, it is not likely."

TIA!!

Another clue would be helpful, cause the only general side effect coming up is photosensitivity. Did MD specify the chemo drug or drug regimen combo?

Radiation side effects? sm
Subject: Radiation side effects? sm

This doc is difficult to understand, but I'm pretty sure he's saying this and just want to verify.

Pt had Hodgkin's disease secondary to thyroid cancer.

On physical exam, head is normocephalic but smaller in proportion to the rest of his body. This is not new after his radiation treatments.

Is anyone aware if radiation has this effect? Thanks.
chemo vs tumor side effects
Subject: chemo vs tumor side effects

Sometimes the suggestion of chemo side effects can also be the symptoms or manifestations of the tumor itself. Temodar (brand name for the generic temozolomide) is primarily used in the treatment of brain tumors, i.e. gliomas, melanomas, astrocytoma, blastomas and the like. Photosensitivity is commonly correlated with gliomas and melanomas primaries as well as brain metastases....especially as these tumor types progress. Also, being a brain tumor, the location of the tumor itself (rather than its type) could be playing a central role in producing that symptom.

This doc has a talent for moving away from the microphone just when he says something difficult...  It sounded like 'supersatorial' side effects.

Dictation:  She had __________ side effects from Neurontin and also stopped clonidine.

I cannot find 'supersatorial' or anything close. Any ideas?

Subject: With nystagmus, the eyeball might "jump." It beats from side to side, or middle to side, etc. sm

Doubt antistigmatic--not a word. Astigmatism is something entirely different. There is no reference in Stedman's ophth for antistigmatic, but do believe they mean nystagmus for "eyes jump." Can you leave a blank? The author apparently doesn't know the correct word to use. Maybe the context of the sentence will help you.

I don't think this is logical. They start at the rectum, then meet the sigmoid (left colon), next transverse, then right (descending) colon, then cecum last.

But I'd be more comfortable with the term rigid sigmoidoscope than colonoscope. No way can a rigid instrument get all the way around, right?
