Not sure how I would format a sentence like this.
The patient has a more-than-20-year history of neck and low back pain.
Would this be correct? Thanks for any help that could be provided.

On your sentence, I would take out a couple of the hyphens:
The patient has a more than 20-year history of neck and low back pain.
