something is wrong here which is throwing you (and us) off. I do OPs exclusively full-time, and never heard of a "four-bundled graft." Try again, and maybe we can come up with something for you.

I have a name that I can't understand in the document anyway (person's last name) and I can't find it to verify spelling - so it's got to go to QA anyway, I come back here and post if/when QA gets it in case someoen else ever has it. TY for those that tried looking also.

Doc may have meant take 2 tablets in the morning and said the q just as a habit. I would certainly question having to take Lexapro at 2 a.m. In my opinion, don't guess - flag it.

It wasn't locked.  Sounded like lap or lapped.  Doctor was running words together and even made up a few of his own!  Gotta love it !

I have a doc who says stuff like that all the time...give a list of "this is normal, that is normal" only he says the same thing both times. I don't change it, I just flag it.
