Doing a shoulder arthroscopy - - "There was ossachromanali???? noted and this was then additionally noted and confirmed arthroscopically.  This was a very large piece.  It measured approximately 40 mm at a later stage and was unstable to palpation and motion.

when QA changes my CO2 to cO2 in chemistries, it's already on it's way to the client.  They changed it on their own without a QA marker being placed.....

In the hospital I use to work, the surgical department had many subdepartments three of which were called Supply, processing and distribution,  These areas were labled within surgery, I never really knew what each departments responsibilities were because I had never seen it in a report.
