This is bone survey for pat. w/history of multiple myeloma. Need help with this sentence....

In the spine, there are scattered degenerative changes apparent.  s/l Rarely prominent of said degenerative changes on the right at L5.  Cannot tell if he is saying Verily, Rarely, Barely?  Can anyone help with what would be normally used in this case?  Thanks.

Some carotid artery calcifications in the left carotid area are apparent. Degenerative changes in the thoracic spine and lumbar spine. There is disk space narrowing involving the L2-L3, L3-L4, L4-L5, L5-S1 levels. I do not appreciate any obvious s/l levique changes in the cervical, thoracic or lumbar spine.

There may be an L, as is lycogenous or gly..the only part that is really good (which the whole dictation is difficult to hear) is the "cogenous"  Thanks for your thoughts!

tooth shaped from injury or what have you.  And like I said 9 times out of 10, odontoid refers to the "toothlike odontoid process in the second cervical vertebra."  That's pretty specific, dontcha think?  I've never heard a doctor refer to an "odontoid" in any other part of the body.

Can someone show me an example of how to type a report that has multiple problems. The doctor does one problem completely, then goes onto the next one.

It doesn't look right typing HISTORY OF PRESENT ILLNESS, PHYSICAL EXAMINATION twice throughout the report.
