Not sure this is what I'm hearing.  The patient has an enterocutaneous fistula.  Says "need for daily wound care discussed.  Will check an "in-helm" INR. 

Clues??

I am sure he MEANT to say *tubal ligation* but perhaps couldn't think of the word at the moment and said *vasectomy* - If it's verbatim, I'd put quotes around it 

or a portion of it specifically to induce sterility.  It absolutely is not correct to leave vasectomy in a female patient's report.  It is wrong!  Nowhere in any of my reference books can I find vasectomy referring to anything other than the vas deferens for male sterility, NOWHERE.  I don't care how you break up the word, you can not change its meaning as a whole.

The QA person who filled it in is a boob who is simply filling in blanks for the sake of sending a report with no blanks.  The report simply should have been sent with the blank and a note drawing the physician's attention to the discrepancy.

In cholecystectomy, Dr. uses Endo Clips but what is the correct way to transcribe "the duct was endo-clipped"?  (Endo Clipped??--just doesnt look right).  Thanks so much for any help.

Sentence is: 

The patient then had an episode of ventricular tachycardia that was short-lived and received amiodarone 150 mg IV on a S/L "drip clip."  

Talking really fast, this sounds right to me, but wanted to verify??

The patient shut his thumb in a car door.  The doctor wants to drill a hole in it.  He dictates:  "Using a heated paper clip, a reasonable sized hole was drilled....."

Is paper clip correct?
