I did check a fasting lipid profile on her today and a chem 18.
She has hypothyroidism, metabolic syndrome, glucose intolerance, and hypertension. Any clues?

 chem-18
 Subject: chem-18

It's a chemistry panel.  There's also chem-7, chem-14, and chem-20, probably others.  For instance, the chem-14 is the CMP and does 14 chemistries:  glucose, calcium, albumin, total protein,  sodium, potassium, etc.  I don't know offhand what all is in the chem-18.
 Chem-7
 Subject: Chem-7

 Good Question!

I use Chem-7 because this test is the same as SMA-7, and I find the SMA tests listed with hyphens in Stedman's Abbreviations, Acronyms, & Symbols.

I would think that any of the 3 ways would probably be okay to use unless your client/employer has a preference.
 I use Chem 7.
