I would separate this from the social history with a new heading unless you're typing an account that is absolute verbatim and no embellishment is allowed. 

I try to think of the record from the patient's standpoint - the easier the information is to locate quickly, the faster and more efficient their treatment will be in a pinch.  I know I'd want my record treated that way! 

I have never come across this before...  my cc line is "Karen Arndt at Buckley Mendleson & Criscione, Attorneys-at-Law"

Problems: It wraps to the second line and I don't know how to format/word that sentence into a cc line.

Helllllllllllllllllllp!!!!???? 

I would type it as follows: aminophylline, Lanoxin, 25 mg, Lasix 40 mg, and Cardizem.

Looking for any help on formatting transcription labels in Word to print to a laser printer.  We purchased labels (3 per page)...I have tried to change settings in Create New Label; however, if we need it to continue on the next label it doesn't seem to work, the text continues, but "underneath" the next label.  How can we set it up so that it continues on the next label?  Any ideas would be greatly appreciated!

The patient was seen by Dr. John Smith of the cardiology service.

OR

The patient was seen by Dr. John Smith of the Cardiology Service.

This always messes me up!  I've done it the second way for many moons, but I'm training a new girl who says according to the BOS, I'm wrong.  Just wondering how everyone else does it.

I can't remember anything these days!  Please jog the memory of a scatter-brained mother of 4 young children by telling her the answer to the following question:

Do you italicize the word monilia?  Does the word need to be capitalized when transcribed within a sentence?

How should you format the following:

Can anyone help me do the following:

I want to do a numbered list (i.e. an IMPRESSION: section), so that:

The numbers are flush left again the margin; and -

Each entry is indented a few spaces (a few spaces to the right of the numbers in the list).  If one entry in the list goes for more than a single line, I want the second line of that entry to ALSO be indented -- so that it does not sit underneath a number and does not look good. Every time I try to use the MS Word numbering feature, it pushes the whole list (including the numbers) to the right, so that the numbers themselves are no longer flush agains the left margin.
