For future reference, depends on how it's used.
Subject: For future reference, depends on how it's used.

Per Gregg Reference (of grammar, usage, style, etc.), is lower case--master's degree, bachelor of arts degree, doctorate-- when used generally like in your example.  But capitalized when used as a title, e.g., Jane Doe, Doctor of Philosophy.

Glad you asked.  I needed a reminder too

Could "Overreactive lispossum" be "overreactive specimen?"

Originally, I was thinking "overreactive splenism" which I would edit to "overreactive spleen" but diagnoses are usually NOT listed with lab values, except maybe as a statement "consistent with..." so that probaby would be wrong, although abnormal cells listed are found in hypo- and hypersplenism.

In any case, just thoughts for future since this is a day or 2 later than original post.

Also, I saw serocytes in lab terms, and assume it should be "spherocytes." At one of my very first temporary transcription jobs at a laboratory, I had mispelled "spherocytes" and "schistocytes" and doctor whose report I typed had me come back to his office and look at slides to see the cells so I would remember in the future.

Have a great day.

Subject: "cc" is a possible future listing in JCAHO "do not use list" sm

JCAHO requires its members to follow an official "do not use list" and "cc" is being considered for possible future inclusion in the JCAHO "do not use list" to be replaced by "mL" because it can be misread as U or unit when poorly WRITTEN...typing it is not as problematic but I go ahead and change it to mL to encourage dictators to make the "mental shift" away from "cc" ...hope that helps! see this link http://en.wikipedia.org/wiki/List_of_medical_abbreviations

Subject: For future reference, have reports similar to this quite often. I do transcribe them verbatim, this way there is no finger pointing on what we as transcriptionists take as a "liberty" in interpretation.

If it is ever in question, they can go back to the dictators original dictation (if available) and see that is what he/she said and they are directly responsible for the content of their reports.

Hope this helps.

Below is the paragraph.  Cannot find s/l Nuez, news?

The patient would like to get his s/l Nuez system again.  This is basically an erectile dysfunction system that I have prescribed before.  It is an older system, but it worked very well for him.  I would much rather him do that than use an oral drug.  So I wrote him for that today as well.

Thanks in advance.

Subject: Which system?

Sorry for such a stupid question that I should know, but I am drawing an absolute complete blank on this one (just one of those dementia moments)...In physical exam what system would I list this under.  I'm drawing a blank thinking maybe genitalia, pelvic, rectal or what (none of it looks right to me).  I've been sitting staring at it for about 10 minutes now and have just confused myself. This is the info...

Penis is uncircumcised.  He has a 16-French Foley catheter in place.  There is clear urine output.  Testicles are descended bilaterally.  His prostate is 50 grams and benign.  He has normal sphincter tone.

Thanks much
