JCAHO requires its members to follow an official "do not use list" and "cc" is being considered for possible future inclusion in the JCAHO "do not use list" to be replaced by "mL" because it can be misread as U or unit when poorly WRITTEN...typing it is not as problematic but I go ahead and change it to mL to encourage dictators to make the "mental shift" away from "cc" ...hope that helps! see this link http://en.wikipedia.org/wiki/List_of_medical_abbreviations

Per Gregg Reference (of grammar, usage, style, etc.), is lower case--master's degree, bachelor of arts degree, doctorate-- when used generally like in your example.  But capitalized when used as a title, e.g., Jane Doe, Doctor of Philosophy.

Could "Overreactive lispossum" be "overreactive specimen?"

Originally, I was thinking "overreactive splenism" which I would edit to "overreactive spleen" but diagnoses are usually NOT listed with lab values, except maybe as a statement "consistent with..." so that probaby would be wrong, although abnormal cells listed are found in hypo- and hypersplenism.

In any case, just thoughts for future since this is a day or 2 later than original post.

Also, I saw serocytes in lab terms, and assume it should be "spherocytes." At one of my very first temporary transcription jobs at a laboratory, I had mispelled "spherocytes" and "schistocytes" and doctor whose report I typed had me come back to his office and look at slides to see the cells so I would remember in the future.

For future reference, have reports similar to this quite often. I do transcribe them verbatim, this

If it is ever in question, they can go back to the dictators original dictation (if available) and see that is what he/she said and they are directly responsible for the content of their reports.

You had a great question, and you are going to be a great MT.  If my response sounded a little cranky, I did not mean to be.  My point is after 25 years, I am not in the business of practicing medicine, and my inclination is that the doc, being a good doc, removed the colostomy bag and examined the colostomy site; and what he actually meant was, "no blood coming from colostomy site".  But to add the word  site or bag, when he said neither, gets into the business of practicing medicine and that is not our business. The best thing to do is put the responsibility back on him and go verbatim.  You are going to be a great MT, I worried to death about things exactly like this 25 years ago.  Good luck and come on the board anytime, plenty of people helped me, and still do.
