Upon rectal exam, the patient has 1 "spline" or "swelling" right at 12 o’clock. Upon digital exam, she has no internal hemorrhoids palpable. "Urine negative. Hemoccult." Her urine dip today revealed 1+ blood, 1+ protein, positive nitrates.


I can't tell what she is saying, sounds like either "spline" or "swelling." Spline doesn't make sense. Also, it didn't make sense where she dictated: "Urine negative Hemoccult." She doesn't dictate punctuation so I don't know how to make sense of it.
