The RN states "

White count on 03/31/2009 was 3.96. A and C is 3100, hematocrit 32, platelets 113.

Do you think A and C refers to hemoglobin A1C or is there something else I am missing given the value of 3100?

I would appreciate it if someone could read this and give me some feedback as I am not sure of the one word in this section of the physical exam.

Slightly enlarged prostate.  It is unchanged from the exam described a year ago.  He still has two small concretions? sounds like conprecions?   which appear at the right and left lateral prostate but not seemingly contiguous the prostate itself.
