Doctor is giving clinical history for an MRI of the thoracic spine and it sounds like he is saying "braying" of skin under breast area.

Any suggestions would be much appreciated!

s/l erutus q.i.d.  If this helps, the patient has microalbuminuria, hypertension and hypothyroidism. 

sounds like profenol or could sound like: propanthenol, propamfenal

for treatment of headaches.

The patient was referred for a left thyroidectomy  which showed benign, hyperplastic follicular nodules, s/l mild field of focali, moderate chronic thyroiditis with focal Hurthle cell metaplasia consistent with Hashimoto’s thyroiditis.

Does the mild field of focali make sense??  He is difficult to understand.  Thank you in advance.

I know patient returns for "a" followup- but what about patient returns for "followup" or "follow up" of bursitis, etc.- need this ASAP- thanks

Has anyone heard a surgeon say "the room was warmed, the patient was prepped and draped" - this is for a patient with hemorrhagic shock and about to undergo thoracic surgery-

Doctor sure sounds like he is saying a dosing of b.a.d.  I cannot find anything to verify and have never come across this before anyone?

medication s/l "absorbabond".  all i know is that he takes it three times daily.  sorry for lack of info.  any help is greatly appreciated.

What could this be?  s/l Kaylatol 40 gel; she can put twice a day to her nails.

I don't have much info to give on this.  It is a very short report.  The only illness mentioned is pneumothoraces. 

The patient is on Valium and (s/l) __betrasept.

S/l **May-gay suspension 400 mg per tube daily.

This patient is anemic, hypotensive, and has squamous cell carcinoma of the epiglottis. I am not findings this one. Thank you

Patient with history of renal transplant and diabetes mellitus type 1

**Cy-a-lim-ose** 2 mg p.o. daily

s/l Sta-lic-s 120 mg t.i.d.

I don't have much to go on other than his diagnoses (these are at home medications): left lower lobe pneumonia, diabetes type 2, hypertension, COPD, and glaucoma

Sl/declotapine.  The ptatient has hypertension, microalbuminuria and hematuria.  TIA

Under list of medications, sounds like Sotic 10 mg p.o. q.d.

I have no way of knowing what this medication is for. Past medical history is Diabetic with neuropathy, hypertension, hyperlipidemia, history of left knee arthritis, osteoarthritis, and also with hypopituitary with steroids, COPD, home oxygen dependent.

Under assessment, doc is stating "Hypertension stable on al-ti-za"

She is an ESL, and I think she may be saying Diltiazem, but wanted to get other opinions. Thanks for any help!
