The patient has tenderness throughout the Achilles tendon from the musculotendinous junction to its insertion site.  He does not have any pain over the ____ (SOUNDS LIKE 'OWLSCOWLISIS' OR 'ASCOWLISIS').  There is not any restriction in dorsiflexion, plantar flexion, inversion, or eversion.

I have looked everywhere online - articles and pictures with diagrams - the only thing I've found that comes close is hallucis ... but what is he saying before it?  It doesn't sound like abductor, flexor, or extensor.

Any suggestions???

Does anyone have any idea what would be injected into a foot for a Mortons neuroma, sounds like sil______ LA. It is injected with Dexamethasone as well.  Thanks. bb

Any help on this would be greatly appreciated:

The patient enters with discomfort on the medial borders of the right and left hallux nails. There was a slight incurvation and erythema with these areas. An incision and drainage of the offending borders was undertaken. A bedioguin was applied to the affected areas. He is to keep it in place for approximately twenty-four hours and then reevaluate it. If he has any problems, he is to contact our office for further treatment.

Does anyone have an idea as to what this might be? It also sounds like pedioguin or maybe podiaguin? I don't have a clue!!

This dictator is saying he is injecting Dexamethasone and ? Selicinistra?? Got any ideas?

Patient has an ingrown toenail.

ASSESSMENT: This is a 75-year-old female with *s/l* onycocryptosis.

I'm lost, please help!

Dr. says:

We may consider a new accommodative device with dell in the shell as well as increased top cover cushion.

Help!! podiatrist dictating s/l mastraunaut of Henry???, taka pallic position and Hope's tonsil. I have been doing this for a long time, can't find these anywhere.. Any suggestions???

This is an OR.  Dictating what s/l "triller" oscillating saw. 

Also, Osteomed antibiotic beads.  Very clear but unable to document anywhere. 

Dx foot pain/ Morton neuroma..

S/L Hy-Grins deformity.

Doc is doing Austin osteotomy, arthrodesis, met head resection.  She's saying (what I believe is during the arthrodesis but I'm not sure - she just closed the osteotomy site) "The area was copiously flushed, the capsule was then HOURGLASSED??? utilizing 3-0 Polysorb."  Has anyone every heard of that?  Thanks in advance!

Is this correct: 

An ankle tourniquet was applied s/l 1000 drape over it.  The foot was elevated for three minutes.

I am typing a report for a podiatrist, and he mentions s/l luko nail sore to pressure. Any ideas?

It's on the tip of my tongue but can't get it out for the life of me!!!

There is good color and warmth of the feet.  S/L Lob's (lahb's) exam shows lots of protective sensation in the tips of the toes.

Doing surgical report --sl mcglammory elevator can't find it in any book

It's a bunionectomy and she says a few times that there was a scab/sky of cartilage that she removes.

Doc says:  "We could do an osteotomy to correct the hallux interphalangeus or just an excision of the lateral nail fold and nail plate, nail matrix.  Kind of a _______ (sounds like high-fits) type procedure."

Any ideas? 

I am not sure on any of this so I just typed it all as I hear it...any help appreciated...assessment is mid foot arthralgias

Casted today for functional orthotic appliance with devices to include colpolaru mur 4 mm base, matty toron 1/8 inch mid layer to the MTPJ, top cover 1/16 inch full length accomodative heel cutout.
