how would one transcribe "novolog sliding scale greater than 150 3 6 9.  Also dictated earlier was sliding scale +1 +2 +3.  must be some sort of pharmacy code.  TIA!!

The pharmacist confirmed and told me how to spell it. I asked her if it was capped, and she said, "I imagine so...it's a medicine."

This is what I did when something like this comes up. I know they want you to use the slightest amount possible, and usually never prescribe more than a small tube at a time. This is a very huge amount. I find pharmacists usually are very helpful if you don't ask them more than a few times a year. I would rather do this than say "a pound," just picture it in your head - that's a lot.

dictation:  I inquired with the respiratory therapist to see if they had Advair, which they did not.  We did not have it in our s/l Pixus.  I can find reference to some sort of automated pharmacy something but do not know if it is capitalized, all caps, initial cap.  Anyone heard of this before?  Thanks a lot.

I had never heard of this myself so I looked it up on the Internet, and it is really interesting how they do all that work ahead of time and get things rolling, and then are ready for the patient coming in. 


My thought would be Code STEMI as long as it was not in the diagnosis, and if that is the way the doctor dictated it.  If he dictated that whole thing out, then it would have to be expanded, but Code STEMI sounds good to me.

I am not sure how to transcribe CPT codes with modifiers. Doctor dictates "CPT code 15734 with a modifier of 51." Do I transcribe it as above, or is there a standard format for adding modifiers?

Anybody know the correct way to type this?  It's a billing code dictated in the progress notes, which is done sometimes on one of my accounts.  I've searched the web and found it spelled every way imaginable, the billers and coders board here looks abandoned, and search of boards turned up nothing.
