This is a patient admitted for a hysterectomy and salpingo-oophorectomy. Here is the sentence that makes no sense, this is an ESL, too.

A focus was then placed ** after careful** level of the angle of the mandible, probably to presented dental disease or some mandible node.

The sentence before: A PET scan showed the primary FDG-Avid located probably in the cervix, and no evidence of regional metastatic disease, no evidence of distant metastatic disease.

Thank you for any help with this mess.

Certainly, if there is any kind of stricture or lesion that might improve his nutritional status, this certainly could lead to his improved comfort and increased survival.

The pt has dysphagia and has lost weight, so doc is sending him to an ENT physician.

It would make a lot of sense given that she is elevating her hand. I tried those keywords you gave and still pulling up nothing. However, it really sounds like she is dictating "Carter pillow" but I had never heard of one before so I really wanted to double check. That most likely has to be it but I'm still debating putting a blank. I'll listen a few more times and most likely use that. Thanks so much! :)
