Wasn't sure which board to put this on.  I have 20-some years exp in onc, can usually Google to find new terms/meds etc, rarely have to leave blanks.  One particular dictator (not a doc, a PA-C who is not only a mushmouth but also mispronounces everything) today stated that the pathology showed something-cells suspicious for carcinoma.  It really sounded like he was saying "palliative" cells, but of course that doesn't make sense.  I tried to make it into "palisading" cells but no, it definitely ended with "ive" not "ing" so I sent it to QA.  I went back in the report later in the day and sure enough, QA had filled it in with "palliative" cells suspicious for carcinoma.  What would you do?  Please correct me if I'm wrong, but this doesn't make sense at all to me.  Even though it did sound like "palliative" cells, I really think he meant something else but was mispronouncing it.  Any input?

The RN dicatates the dosage as 20/k at 84 cc/hour.  Is this how I properly transcribe it and does that mean 20 mg per kg?

"She was positive for opiates on her screen and negative for (s/l six-mono-asteo) morphine. She had an elevated QTC and an elevated amitriptyline level"

You are right, there seems to be a discrepancy in administration route, if that is exactly how it was dictated.  With a touchy person, I might try:


Please review possible discrepancy in route of fentanyl administration. 


Something like that.  Polite, yet not frankly pointing out a mistake, just saying a mistake might be a possibility, up for him to decide with a review.  Hope that helps.

well, who should care?  The MT him or herself.  This can be a crutch IMO.  And, no, my opinion doesn't amount to a pile o' beans in the grand scheme of things.  Maybe my post should have been more helpful, than critical, and should have asked posters if they know how to search.  I've gotten really good at it and can find just about anything really.  For instance, someone just recently posted ligament of Treitz question.  All I did was google, ligament of *.  Bingo!
