 The patient was found unconscious and brought to the hospital:
 
 "She was positive for opiates on her screen and negative for (s/l six-mono-asteo) morphine.  She had an elevated QTC and an elevated amitriptyline level"

 Probably too late for this message, but wanted to give you an FYI - I have several doctors who say "fifty, five zero" hoping  the transcriptionst understands they are saying 50 rather than 15
