I'm transcribing a TAHBSO, and the doctor says there is a small s/l Allen Masterson window noted in the posterior cul-de-sac. 

Could be Ellen instead of Allen or Alan or Allan.  I've googled them all and can't seem to document this. 

Attention was then turned to cannulating the s/l GATE or GAIT?.  This was done with the assistance of a C2 catheter and a Glidewire.  Once the GATE or GAIT? was cannulated, this was confirmed with the pigtail catheter and the proximal device.  Thanks.

it was noticed that he was having               drainage coming out of his wound consistent with an enterocutaneous fistula.

The word sounds like bilgus or valgus, I am not sure, something like that.  I can not figure out what the word is.  Any help is greatly appreciated. 

The sentence pertains to a small bowel resection and hernia repair.

Can anyone please tell me the correct way to write wound vac?  I have seen it as Wound-Vac, Wound VAC, wound V.A.C., etc., and have had it corrected in QA all of these different ways and then some!!  Does anyone know who makes these so I can look at their website?

I don't know if *sevil* is really another word, or if the doctor started a word, got mixed up and then said it right. Please help!!

It s/l the dict is stating the chest is hyperesthenic, but clearly not stating hyperexpanded-- not sure.

I don't get these much and there is a new doctor dictating. 

I question "Cardiac size and cardiovasculature" and also have a blank. 

"Cardiac size and cardiovascular" are normal.  No infiltrate or pleural fluid is seen  ___________ trachea is midline.  There is no hilar enlargement.

chest x-ray showed right upper lobe mass, suspect lung s/l nee-av or lee-av???

Chest x-ray.

Marked elevation in the right hemidiaphragm with s/i savagies and atelectasis and/or scarring.

There is may be a trace of bronchiectasis within this region. s/l Upuvenier density is noted elsewhere.

Any idea what above term is? Thanks

the dr. dictates - A small periumbilical incision was made, subcutaneous tissue was bovied through and the s/l "point" or "bone" was dissected down identifying the umbilical stalk.
