yes, I can hear vertigo.

Posted By: Joanna on 2009-06-25
In Reply to: What is the exact sentence or dictation? - pc

Subject: yes, I can hear vertigo.

But now the sentence is more jumbled!! What I've already transcribed is this:

ENT: There has been no tinnitus, ______(dass?), or vertigo. _______ (s/l no-known??)

The very last term seems to be at the end of the EYES subheading as well. I can't make it out.

Doctor spells it as "Zlopapine"  ;o)  I have looked at generic ambien, zolpidem and do not think an insomnia drug would work.  I have had this drug come up in a couple different reports as well and it's driving me crazy that I can't find it.  I have tried Z and X beginnings.  Thanks for any help!

Pt with vertigo with s/l "a lith a tory" or with orthostatic changes. sm
Subject: Pt with vertigo with s/l "a lith a tory" or with orthostatic changes. sm

He states that he has lightheaded sensation and vertigo on change of position of the head, whether it is __ or with orthostatic positional changes.  (Later in dictation, doc says "His dizziness is associated with head position.  However, it is both with head turning side to side, as well as orthostasis.", so the word seems to be related to the turning head maybe??)

Since there are so many different transcription platforms, you can't always program in the 1/2 symbol and typing it like that - 1/2 - is clumsy and can be unclear when read. The guidelines at my last in-house job were to convert it to decimal if possible.
