A colon may be used in place of the verb to introduce either type of list.

Past medical history: (1) diabetes, (2) cholecystitis, (3) hiatal hernia.

That's all I see regarding the word include and lists. Basically it was the point and not necessarily regarding medications and the word include. It was more regarding common sense and good sentence structure and formatting and grammar.

Medications:
1. *Include* Zocor.
2. Ambien.

C'mon now. And thanks for proving another point; that most times you can't get your point across the way it is intended when trying to explain something like this :)
