Hi!  I can totally relate to your pet peeves for all of them except one (see below):

MEDICATIONS

*Include:*

1. Zocor.

2. Ambien. 

I never used to include the word "include" until I was dinged on it.  The QA person told me to always include it if they dictate it as it is their (dictator's) way of saying, "include but not necessarily exclusive of:"...sort of a CYA.
