Why do some MTs type stuff like this.........

MEDICATIONS

1. *Include* Zocor.

2. Ambien.

or

MEDICATIONS

*Include:*

1. Zocor.

2. Ambien. 

Or

FAMILY HISTORY:

*Is* unremarkable.

And I don't care if it's verbatim.  If MD said "diagnoses are hypertension and CHF" would you type:

DIAGNOSES

1.  Are hypertension.

2. Congestive heart failure.

or

DIAGNOSES

Are:

1. Hypertension.

2.  Congestive heart failure.
