P.S. Miami may be one of the largest cities, but it is still just one city located in one small state. There are 49 other states. Miami is just a small speck when you compare it to the rest of the United States.

I live in South Florida and you may as well call it a foreign country. I asked a clerk at local Walmart for Phiso-dem...she had me write it on her notepad she carries around, because she doesn't speak English!!!!

America is a melting pot and English is not and should not be the national language. I'm Native American and while I'm not happy about the influx of immigrants - which I'm sure your ancestors were - America is a melting pot and at one time English was the 'second language' around here and others had to adapt.
Deal with it. America is a safe haven and refuge for other countries. Why was it okay for your ancestors to come here but it's not okay for others to come here?
At least most of them speak English as a first language, eh.
Except the Francophones/Quebecois. Plus, they're not undercutting us on wages.

Assault on the English Language
I transcribe for a doctor who I think may be Russian, but anyway, he has lived in this country (USA) for many years and has not taken the time to learn to speak succinctly. As a matter of fact, he seems to be dyslexic on top of the heavy accent and often phrases words backwards, does not know how to speak concisely, and I am thinking whoever has the misfortune of reading his HNP's is going to think he is a real Dip (even after editing). Even our editors have a real challenge getting through his work. I wish I could shake the snot out of him and require that every ESL physician practicing in America be required to speak the English language fluently.
ESL just means English as second language.

I know what you mean - I cannot believe AAMT can change the English language-
but they dooooooo!!! It's so frustrating. I absolutely refuse to type one space after a period or question mark. I was taught two spaces from TYPING CLASS and that is the way it will be.
Personally, I think it is so the MTSOs that pay for lines and spaces will not have to pay for extra spaces. But that's just my opinion.

I do. This is from the American Heritage Book of English usage in regard to collective nouns. You see, I also have a degree in English and also taught. I do know the rules for collective nouns.
"Some nouns refer to a group but are singular in form. These nouns are called collective nouns. In American usage, a collective noun takes a singular verb when it refers to the collection considered as a whole."
Last time I checked, even MTs use the English language and we should have a command
of that language and that includes all the rules and regs taught from gradeschool on up. Now, it's totally up to you if you choose to be an AHDI robot. I acknowledge that a lot of MTSOs use the BOS as their guideline and if you work for those MTSOs, then you are obligated to follow "their" rules if you want your paycheck, but that does not mean that the rule book is correct. Quite simply, it is not. A group of retired MTs got together, had a meeting, and decided to make up a set of rules. Then they had a really fantastic marketing team that set out to market those rules as the end-all, be-all of medical transcription. They definitely found a willing audience for the crap they were peddling and they capitalized on it. I guess that's the American dream -- making a bundle of money of a bunch of suckers willing to pay, no questions asked.
I, however, choose to buck the system. I'm a noncomformist, I guess. I earned my bachelor's in English before I decided to become an MT. I believe it's important to be able to express oneself articulately and intelligently using the written word. I dislike what texting, email, and chat rooms have done to the English language (witness above post "wannbe MT ur opinions..."!!!!!).
Oh well, that's just me. I work for a hospital that could care less about the AHDI and its BOS. In my spare time, I wage my anti-BOS and anti-AHDI campaign in the hopes that MTs will wise up and MTSOs will too.

MQ acts like ASR will be all they need and forget people with English and
their hospital clients are watching. When there is no pool of MTs to get anymore when MQ has destroyed the competition and driven what is left of MTs to other sources of employment, well-------------------------maybe there won't be hospitals any more, who knows. The way our country is going I guess we will have to be treating ourselves from over the internet, LOL.
But, back to being serious, I see more and more hospitals advertising for MTs to work for their own individual institutions, and not using transcription companies.
I hope the pendulum is swinging back to hospitals having more control of their records. 
Otherwise HOSPITALS are in for a big surprise. Wasting Mi$$ions on MQ's and Philips "voice recognition" gadgetry is throwing good money after bad. The "suits" have complicated creating a medical record so much, it's literally a joke. 
But that's what big business does isn't it? Get "suits" into a situation and have them try to make it a "profit making venture" out of it - and pretty soon the whole job gets lost in the hubbub and all you have left is confusion and dollars flying around, and the product is an unattainable goal.
Oh wait, but you will have a NEW COMPUTER SYSTEM that is supposed to do everything but wash dishes, and the suit will walk away with another of your $23million dollars.   
I noticed the last time I read stats from MQ on how many MTs they have it was (drum roll here)............10,000. Remember when they crowed they had 13,000?
Was this "Frank's" plan when he took over?  Or are we all jumping ship because they are an intolerable place to get a pay check? You do the work and even then they want to renig on paying you for what you did.  NOW THAT's REAL CLASS, not.

Up to 20 Dead in Fire Aboard Evacuees' Bus

A bus carrying elderly evacuees from Hurricane Rita caught fire early Friday on a gridlocked highway near Dallas, killing as many as 20 people, authorities said.
"Deputies were unable to get everyone off the bus," spokesman Don Peritz said. "We believe it's going to be closer to 20 fatalities.
The bus, carrying about 45 people, was engulfed with flames, causing a 17-mile backup on Interstate 45, already heavily congested with evacuees from the Gulf Coast. The bus was reduced to a blackened, burned-out shell, surrounded by numerous police cars and ambulances.
There were indications that oxygen used by elderly evacuees could have had a role in the fire, Peritz said. There were a series of explosions, apparently from the oxygen equipment, he said.
"The early indications are this is a mechanical issue. The driver did survive the accident," Peritz said. "It's my understanding he went back on the bus several times to try to evacuate people."
Interstate 45 stretches more than 250 miles from Galveston through Houston to Dallas. The crash site is roughly 17 miles southeast of downtown Dallas.

The way people appear in public anymore--
Reminds me of a T shirt I saw that read-- Too many freaks, not enough circuses. I think slippers are the least of your worry. I think people should at least be covered up and not letting everything hang out for public display. If they want to look like that at home, but not in public.
I just ignore people anymore...they don't bother to listen anyway---LOL
It is hard when you have to take care of the kids (mine are 8 and 14 months), work and somehow manage to have your house not look like a disaster area...sounds fun to me---LOL...in all seriousness though I love my job and that I am here for my kids and I don't have to justify that to anyone...

If that were the case with 30+ years, my per line pay would be more than what I make now. No more pay for experience.
You know, I dont think people care anymore what MQ offers or doesnt offer, I think everyone is just
sick of all the confusion, dishonesty, sneakiness, etc. You never know what is around the corner so after this there will be something else no doubt. I think I and most other MQ MTs just want a company that they can respect and trust. MQ can offer this package and turn right around and change that. Leopards dont change their spots. It seems like they are intent on running this company into the ground and they are doing a good job of it. I dont see how they ever will stand up against the money they owe all of these hospitals. You still have to wonder what prompted them to send checks out to the DQS MTs for mistakes in calculation of lines. Give me a break. Maybe they are trying to settle lawsuits to get out of the MT business and focus on just technology. They sure dont know what they are doing with the MT side for sure. I think they felt they had everyone up against a brick wall but as accounts leave so will MTs or MTs will just move on to another business as the wages get lower and lower so then all the work can go to India. Older MTs will cut back or retire and younger ones will leave for something else when they see what they have to battle with the work left over from ASR or VR for the small amount of money not to mention EMRs coming quickly.
People have no patience anymore. I gave a $10.00 donation and I got an email the next evening.
I think since these people also have a transcription service, no one should expect an instant response. Besides, it's more about giving than getting anyway.

"people" have decided to group trailer people as trash. that is because there are enough people in that group to earn the title and even if you aren't trash, you are categorized by others. did i think i was trash in lower class neighborhood surrounded by people who drank and fought all weekend? no but i knew i wasn't staying and did not try to pretend that all the fools in the neighborhood were just nice folks who ended up where they were because high horse snobs deemed their neighborhood low class. people for the most part live exactly where they belong because they don't want to educate themselves, they don't mind "trash" around them and they don't want to be bothered trying just a bit hard to extract themselves from that world. they justify everything to themselves i guess saying everyone who doesn't like their lifestyle is a snob and the comedians (Jeff Foxworthy/Chris Rock, etc) who make fun of them are just ill-informed.
As for me, I fought hard to get out and don't even want to look back. It amazes me people stay for generations.

Language skills are definitely at the top - I agree. So many outsiders think all we do is type and memorize a few terms. Makes me crazy sometimes to think we are being looked down upon for, quote - just typing, how hard can that be?

These ESL's speak more than their own language, sm
many are fluent in several languages. They also pay income taxes at the same rate as USA-born doctors. Until you can speak their language as well as they can speak ours, have some compassion for their efforts, and be glad that you have a job. There are ESLs wherever you go. Better get used to it. Or better yet, why don't you travel to a country where you don't speak the language. It might humble you a bit when you see how difficult it is.

AT&T language line
Hospitals can set up an account with the AT&T language line. You call the number, give your account code and tell them what language you need. They have tons to choose from and they are available in real time. You have to use a speaker phone so the patient, translator and care provider can all hear each other. Some 911 dispatch centers use it too.
Sometimes the default language gets (sm)
changed on a document when I am typing and I have to go back in and reset it to English. I notice this when my autocorrect entries do not work and I know that I have them in the file. It only happens every once in a while and I have to "select all" and then change the language back to English. Hope this helps in the future.

learning another language
It isn't that I wouldn't like to learn another language if I had the time. I would. It would be neat to be able to speak another language, but I don't. And it makes me fustrated when immigrants come here and expect me to know their language when they are the ones who came here. They should know proper English. Or just decent English. If I moved to South America I would not expect them to understand me. I would take it upon myself that if I wanted to communicate I need to learn good Spanish. And if I had a job where I did something such as dictating, I would make sure I knew decent English and tried to make myself understandable. Some ESLs act like they don't care if you can understand them or not. There ARE some who try though.

After a certain age, the accent is very difficult to lose. Some people have more of a talent for this than others. I know, makes it hard for the MTs.. but part of the job. Not saying they shouldn't try to improve... I study another language, but I am always going to have an American accent.

Medical Language Specialist
That is our title, Medical Language Specialist, which really throws people. When asked to define what that is, tell them you translate medical dictation to document form, be it hard copy document or electronic docment, to provide pertinent medical information for patients, physicians and hospital staff. See their reaction. 
Of course when they say "all you do is type," ask them to spell esophagogastroduodenoscopy as an example of the terminology you use. That usually shuts them up. (Smile)

ESLs are about 60-70% of the dictators, we as language
With this particular doc did you just guess, or did you leave blanks? How many times did you listen? Did you ask for help? They all basically have the same or similar awful accents, so in the beginning you need to take the time to really listen and get to know the accent, that way in the future you can zip through them. What did you do to make them fire you, what was the mistake exactly?
Here is an example of the communication/language barrier
My ex-husband had the flu, high fever and it was not getting better. He was quite ill. He went to his physician, a foreign physician with heavy accent. My ex tried to explain how sick he was. The doctor rx'ed antibiotic and sent him home. That night, my ex had a high fever and went into a seizure. Paramedics were called. He was still seizuring by the time they reached his home. He came into the hospital with bilateral acute renal failure due to the infection and high temp.
When I spoke to him, he said it was clear the physician did not appreciate how ill he was and the language barrier was limiting their ability to talk to one another. My ex was so ill, he didn't feel like being assertive. He should have been hospitalized. But, that is too late now. His kidneys never regained function and now he sits with millions of others waiting for a donor at UCLA.
Heartbreaking, isn't it?

My take is that you are both correct and mistaken. People with French as their mother tongue can be both challenging and reliable to transcribe in English. The challenging part is the omission of certain sounds that are normal to the English-attuned ear. The reliable part is that the differences one hears are usually SO predictable. I belive that with more info I can help you even with your specific problem.

Had an Iranian who didn't know the language sm
so he would just say, "whatever's that word I want - if you don't know it, look it up in the dictionary." That was between all his ummm, ummm, ahhh, It doesn't matter what country he was from, it is just to get the point across that he had a very heavy accent to boot and he never bothered to try to improve one bit in 12 years. His version of amaurosis fugax was "amagosis fewjacks." Grrrr.
