Hi, I'm new here. EMR replaced about 75% of one of my accounts. They jumped right in 100% but then some of the doctors went back to dictating, at least some of their notes. The system could not provide as much details, plus it took the docs way too much time entering data compared to dictating office notes. Now they make their office staff/assistants enter most of the info, vitals, messages, etc. Still could not make $$$ on the remaining work, now they do it in house. I do agree, once a mistake is in the record, it just goes on and on and on. I know from my own personal experience, grandson has a murmur/biscuspid valve/stenosis, up to this day his records on his exam still say the standard phrase, no murmurs, rubs or gallops.
