Get out now!

This happened to a friend of mine a couple of years ago where the MTSO completely stopped paying her.  The owner of the MTSO had recently purchased quite a bit of land, built a big house, and then suddenly she was claiming that the doctors' offices just simply weren't paying her.  The truth of the matter was that she was in financial trouble and we were paying for it because, not long after she stopped paying my friend, she started paying the rest of us later and later.

I know it's hard to leave accounts that you love and are accustomed to, but get out now.  There's no reason you should have to suffer for the MTSO's poor business decisions.
