I started on Acute care, then went to clinic work, - sm
and then back again. I found clinic work to be more challenging, plus it often included radiology. I think it's more a matter of just getting familiar with each institution's way of doing things, along with new doctors, and getting familiar with a few new terms (which don't we all do every single day, anyway?) than it is one being easier or harder than the other.
DH & I started new jobs & his mom died. Employers couldn't care less. nm

Take it to a blog
No one is telling you not to communicate, just get a blog where you all can meet and "discuss" to your hearts content while your children starve and your bills pile up.  You have our sympathy, now have some sympathy for us and take your whining to a blog.  To me this seems like a no-brainer.  If you need the health coverage, fine, stay, but get another MT job that will pay your bills.  If MQ has no work, working at another company should pose no problems.  But at some point, you have to decide, stay and do something else at the same time to improve your situation or go.  But quit subjecting the rest of us to your pity parties.  Take it to a blog.

I cannot find a local station anymore that plays this type of music. I have asked others what artists played/sang this I really do not know where to look it up at. Could you give me some sources or recommendations. I would love to listen to this music again -- perhaps it would make me young again since I listened to this in my teens and early 20's!

Whiners take it to a blog
I am not advocating to just quit, move or walk away.  If you have read my posts, I have offered suggestions that don't require brash action, just common sense and ambition.  Public boards should be for differing view points on different subjects.  They should not be about pity parties.   It would be much easier for the whiners to have their own blog, where maybe between them they could come up with one good idea as to how to improve their situation.  Leave the public boards for the general public, not a few whiners who only rehash the same thoughts.

What a jolt to reality.  Reminded me of things I'd forgotten I liked about this type of work.  Too easy to concentrate on the negatives day to day.  I have a cat too but he is not as much company as yours seems to be.  Nice spot for him by the window--I'm sure he is very content to hang out with you during the day.

I just took a brief peek, but it looks like a nice site; and I got a kick out of your writing about the ESL doc who ties too hard.

I have a sweet female foreign dictator who ties so hard. I get frustrated when she keeps changing her mind on patients' lab levels, and then I realized she just forgets which numbers are called what. I'm sure it has to be exhausting and frustrating for them as well. :)

Do you have any info about yourself on there? I sometimes read blogs but often cannot find any info or photo of the person, so I lose interest. Guess I just need something to connect with. - Not giving a name (at least not full or real name) is fine, but some info and maybe a photo makes it more fun.

The Death Star of American Medicine




I wish I had written this (JCAHO Unplugged; registration required), because it gives voice to the feelings that I (and probably 99% of the physicians in this country) have about the Death Star of American medicine -- JCAHO. A few excerpts (emphasis is mine):

That is why it really upsets me to watch the JCAHO people walk through my hospital like they were navigating a toxic waste dump. What upsets me even more is the utter paralysis of normal activity that occurs months before their visit and the huge sums of money spent on mock drills and consultants to prepare for the JCAHO invasion. JCAHO (the Joint Commission on Accreditation of Healthcare Organization) arouses more fear in hospitals than MRSA gone wild, and their "visit" has a greater institutional paralytic effect than circulating a neuromuscular depolarizing agent through the ventilation system.

A few unsigned verbal orders, or an anesthesiologist carrying a syringe of Anectine in his or her scrubs, or not locking up I.V. bottles of normal saline will result in conditional or provisional approval, and failure to take remedial action within 30 days may result in the death penalty, which for the hospital means bad PR, and more significantly, loss of all federal money. That's right, you can get the chair for parking tickets in the JCAHO world.

It doesn't matter that the hospital admits 50,000 patients a year, saves countless lives and performs daily miracles. Write q.i.d. twice and you can get your liver transplant someplace else.
"A response by Russell Massaro, MD, FACP, Executive Vice President Accreditation and Certification Operations, JCAHO, follows Dr. Cossman's screed. The only thing I can say about it is that he makes clear that unannounced surveys will be forthcoming in 2006. Oh, joy!

A lot of the press in the past few days has been discussing issues such as oversight, accountability, and clearly expressed legal authority. JCAHO operates without any real semblence of these niceties, and does so with all the subtlety of a bureaucracy run by Darth Vader. Watching hospital administrators lose bowel and bladder control the minute a JCAHO inspection is brought up has always reminded me of Vader's underlings wilting in his presence --- and for good reason. Without the JCAHO seal of approval, they cannot operate an otherwise well-run, caring facility. The organization, as it was originally intended, was designed to ensure a basic level of safety for all hospitals. It has now become yet another "certifying" agency which must come up with new "critical" problems to fix in order to ensure its ongoing existence (those of you who have gone through the most recent mental masturbatory experience of banning QD and QID in orders know what I mean). What is most galling to me, however, is that hospitals must fork over a hefty sum for these frequent torture sessions, and the inspections are done not by practicing physicians or nurses, but by folks who long ago gave up the difficulties of actually caring for patients for the safety of a clipboard to hide behind.

Maybe I have my metaphors mixed up --- JCAHO comes in like the Death Star, but leaves no room for different institutions to solve problems in their own way. The JCAHO mantra can really be translated as "Resistance is futile. You will be assimilated!" As a result, I suppose JCAHO is really The Borg.

Is that concept so hard to understand?  I you cannot understand that simple idea, maybe that is why the lot of you don't have the sense to do something about your situation other than just whine.  Take it to a blog, where maybe between you will be able to help yourselves.  Or better yet, think of it this way,,,  take it to a blog and then you won't have to navigate around the other posts to whine to each other.

Kinship care versus foster care/adoption
Having been placed in a position where I now have custody of my 3 YO granddaughter and going through the legal system, I sought an online network of relative caregivers for children. I would encourage you, especially since you are in Georgia, that if you take any children into foster care with the idea of adopting them, there is federal law that requires the state to take certain actions in a specific time frame. When a child is removed from it's bio parent(s), the state is required to investigate any possible relatives who can take the child before foster care is considered, but even before that, reunification with the parents is the priority. Once a child enters the system and is in the system for 15 out of any 22 months, the state is required to find permanent placement for the child.

The problem with this is that there are case workers who may favor a foster family and do not seek out relative care. I have a good friend in Georgia who had to fight all the way to the state level to get custody of her grandson after the child was placed from the hospital into a foster care home with the promise that the foster parents would be allowed to adopt. She has now adopted her grandson, but it was a long, hard battle to get the state to admit their own interests were placed above those of the child and/or family.

If you get a child placed through the state, please make certain there is not a relative who wants that child before you get your hopes up. The courts are now favoring return of children to relatives even after a child has spent years with a foster family who hoped to adopt them.

States get bonus federal funds by complying with the time lines and being able to close the case, so some states place children in foster care because it is easier than trying to locate relatives.

Didn't mean to go off on a tangent, but I can't imagine my sweet bella going to someone outside her family.

We can also give information who they can contact at their local hosptial to find out if their records are going to India/Asia. Most people don't know to ask for the director of health information or operations manager.

If it was a clinic, it might have been urgent care, but it was NOT acute care. sm
Acute care refers to work in an acute care setting, a hospital, doing at least History and Physicals, Discharge Summaries, Consultations, Surgery notes, Emergency Department notes, and much more, including GI procedures, Cardiology procedures, Neurological procedures, Pulmonary Function Studies.  It goes on and on and it means and acute care hospital setting, not a clinic.

Need opinions

Does anyone use software for transcription that has the automatic faxing and e-mailing so the doctor's office doesn't have to pay staff to do this? 


Was it expensive for you to buy - does the vendor charge you a per line fee or any fee for this - is it easy for you to use - and do you feel that the doctors like this kind of software and its faxing and emailing of transcripts to referring physicians functions?  Also, do you have names of companies you could give so that I could possibly check into this?  I am currently using a phone-in system but I thought an automatic faxing software would draw in prospective doctor clients. 


Any opinions, good and bad, are welcomed.   Thank you.

I'm still firmly of the opinion that it does suck.  I hate it.  The poster who said it's just a mess to clean up, is absolutely correct.  That's the whole problem.  Some doctors are great,  others are so-so, and many are just a mess.  You waste more time backing up and correcting, and could have typed it by hand in the first place with less grief and frustration.


Yes, maybe it is the wave of the future, but as many bad dictators as there are, there are always going to be people needed for them.   Let the ones who love speech rec go for it.  It might help some, saving some people's hands, and boosting production somewhat, but I'd really like to talk to the people themselves who love it and supposedly have great production, rather than just seeing a generic sheet of numbers that the supervisors wave around for show. 


No, it was not the Dragon program, and not a national company. 


This is not on just a few providers, it's about 70% of them in a quite large facility, ESLs included right along with the stumblers and mumblers.


Correct, the supervisors are pushing this program so hard, to save money. Yes, it can be a great help to some, but not to all. 


Medical reports can be extremely complicated, and are in no way comparable to social work reports.  One person, consistently dictating all the time, compared to an assortment of 20-30 dictators/speech patterns at random, with different types of reports?  No, there's no comparison. 


All in all, it does suck to work with.  And if I'm not taken off the program at the end of three months, then I will probably quit.  It might help me somewhat, but I doubt it's going to boost my production that much.  We'll see what happens.  To me, it's not worth the stress and frustration.

need some opinions, please
I applied at 2 companies on Friday evening - heard back from both of them the same day - one wanted a questionnaire filled out, the other said they were very interested in talking to me and asked me when I was available to speak to them M-W of this week - I wrote back and gave times, etc.  Haven't heard anything yet.  If I don't hear by this evening, should I call them?  I'm very anxious to get another job!

I am SO sorry for reposting this. If I knew how to remove my other post from the pressure valve, I would... I do not want to get in trouble for multiple postings, so PLEASE forgive that mistake. I just don't think that the PV gets as much traffic as this board, and I really need some help right now! 


Can I get an opinion from someone who is experienced in this business?  I am a relatively new MT...  I have only been doing this job since March 2005, so less than a year. 


I work as an independent contractor at .08 per line.  My employer is a really nice person, but she will frequently need help over the weekend.  I have always said sure because I want to make myself valuable to her.  However, I have always done as much as I can do comfortably and still have time with my family over the weekend. 


I did an extra 1000 lines this weekend, which takes me about 6 hours because I am still new and research everything if I am not totally familiar with the terms.  I got an email this morning saying that, basically, I had not done enough to help her and she ended up doing "most of it" herself.


My feeling is that she is the owner of this business.  If she overextends herself as far as taking on work and she has to work all weekend to get it done, that's what she has to do as the owner of this business.  If she wants to pay a weekend differential to me, then sure... I will make plans with my husband to take the kids on his own and I will work all weekend now and then.  But if I am working for the same rate, I am not willing to work all week and then work all weekend too and miss out on my husband and children.  Isn't it customary and expected to offer a differential for working weekends and holidays?


I am a bit miffed because I gave up most of my Saturday and an hour or so on Sunday to help her out, and now I am criticized for not doing more for the same pay. Any input from you experienced MTs?


I am sitting on my email reply, but it does have this in it:


"I am happy to help as much as I can over a weekend without a differential, but I will need to do so around my family's schedule."


I need to reply to her email, but am not sure what to say. Thank you in advance for your help.  


 


Opinions please!
Have any of you worked at home for a long length of time and then had to go out to work in the public for any reason? I have been a home MT for a long, long time, 10++ years. I work for a big national, but find that I can no longer sit here and wait for work! I have applied for an in hospital job doing rehab! Is this type of work difficult? Any opinions greatly appreciated. I would really like to have your input!

I got things straightened out, but thanks for your replies. I ended up with a 13-page report right after posting. LOL He spoke clearly, too! It's for a psychiatrist, so it should be interesting to hear those cases.

Yesterday, I received an announcement in the mail regarding the graduation of a long-time ex-friend's daughter's graduation...wheew!


My question to y'all is...the last time this ex-friend and I spoke, we were not on good terms..and actually, she wasn't really a friend.  She was an MTSO that I worked for for about a year.  Turned out (and long story short) she was a crazy person!!!  Anyway, I was very surprised to receive this announcement.


What would y'all do? Send the girl money?...I have nothing against her, though I don't really know her that well...trash the announcement and forget about it, or what? I am by no means going to attend the ceremony, but should I even acknowledge the receipt of the announcement?


I feel like the "friend" just...like my daughter said...went through her address book and sent everyone in it an announcement...probably just for gifts...your help would be so appreciated!

I don't believe I am looking for acceptance or any such thing from my mother, but rather I truly care about my father and aunt's health.  I think sometimes people are very close to a situation and may not be able to see outside of themselves. 


Also, I never said, nor do I believe, that an MT has anything on a NP.  I only believe that we do have some medical knowledge and can sometimes pick up on things.  Years ago my dad had a constant cough.  His doctors (IM, card, pulm) or my mother couldn't figure it out.  I asked if he had started any new medications and he had started an ACE.  He changed meds and his cough was gone. 


I have to think a little more about this, but I do appreciate all the help.


Would like some QA opinions...
I am very frustrated with inability of some of my IC's to punctuate and construct sentences correctly.  Is this a common problem, or am I just being too particular?  Do others correct this kind of thing and inform the MT or just let it go?
